looking for a PHP/Opensource Voting/poll system..

They are doing an awards ceremony for ie best local bar, best local business person etc. they want it so each person can only vote on one category once. ideally by cookie or/and IP address. when the vote is done, it's to say your vote has been accepted and ideally email them thanking for their vote of XX in Category YY Can anyone recommend a script I could integrate?
